Immunization Waivers

Immunization Schedule/Waivers:
Immunization records are required for any new students, regardless of their grade level. Students attending GSRP, Kindergarten or 7th grade need a new or current year waiver.  Information regarding Immunization and Immunization waivers are provided by the
Wayne County Health Department.
